Boulder Community Health is seeking a full-time Cardiac Rehab RN to join our team. In this nursing role, you’ll be responsible for development and implementation of all policies and procedures relative to Cardiac Rehabilitation. This includes evaluation, supervision, and treatment of all patients enrolled in Cardiac Rehabilitation. The Cardiac Rehab Registered Nurse will participate in the establishment of program goals and objectives, quality assessment, problem solving, and marketing. Able to work with patient ages adult to geriatric.
Develops goals and objectives within the framework of the AACVPR Cardiac Rehabilitation program goals and objectives.
Submits budget requests to meet these goals and objectives to Administrator – Cardiology Services.
Develops and maintains policies and procedures reflecting areas of responsibility.
Collects Outcome data and submits to Quality & Decision Support department.
Assists in development and utilization of marketing plan.
Orients staff in areas of responsibility.
Explains programs to others in area of structure and financial management.
Incorporates latest published information and results of research into program.
May be required to perform point of care testing. May be a point of care trainer.
Coordinates operations of the Cardiac Rehab Program.
Assumes responsibility for each patient during the course of treatment.
Develops an integrated treatment plan by assisting patients in goal setting and by gathering objective data that promotes basis for professional plan.
Assures that the patient is involved on an ongoing basis in discussion of plans, goals, status, etc.
Assures that the patient/family is adequately oriented to the program, that the program proceeds in an orderly, purposeful, and timely manner, and that discharge decisions and arrangements for follow-up are handled properly.
Sees that exercise rooms are set up and cleaned after use, including emergency equipment.
Monitors patients before, during, and after exercise according to AACVPR/Medicare patient exercise guidelines.
Refers patients to other services or outside agencies when patient goals cannot be met within program framework.
Evaluates patients as to the quality of care and compliance to program through patient staffing, observations, and surveys.
Complies with cardiac rehabilitation policies and procedures.
Establishes patient goals and objectives. Documents treatment plan & progress in EMR.
Designs and implements teaching tools.
Works with Transitional Care staff, dietician, nursing, and other auxiliary personnel to coordinate patient and family education.
Maintains professional behavior, supports professional organizations, participates in community affairs related to work area and attends continuing education.
Utilizes learning opportunities to achieve identified patient care/management objectives by attending education opportunities specifically related to areas of responsibility.
Assesses self-learning goals, documenting action plan and discussing progress with Health Sciences Coordinator.
Cooperates and maintains good working rapport with all departments at Boulder Community Hospital and Mapleton Center and with patients, families, and outside agencies.
Monitors patient satisfaction.
Manages equipment in patient care area in compliance with operating equipment/directions.
Reports charges in a timely manner.
Demonstrates professional responsibility in the staff nurse role.
Employees are expected to comply with all regulatory requirements, including Joint Commission Standards.
