Provider of critical care medicine/emergency medicine related care for inpatients and ambulatory patients with critical care medicine/ emergency medicine conditions. Is responsible for the continuing management of assigned patients. This includes physical examination and treatment, coordination of medical care with other subspecialists, social worker, data manager, pharmacist, dietitian, nursing and other personnel as leader of a multidisciplinary team reviews. Provider of critical care medicine/emergency medicine will also include operation of quality assurance and utilization programs including peer reviews, open and closed medical record reviews and utilization reviews. Also provides comprehensive critical care medicine/emergency medicine treatments at different settings including the Emergency Department. Maintains an active knowledge of the patient care processes of the Department of Veterans Affairs, the medical center and its clinics. Emergency Medicine physician, duties include, but not limited to: Asses, evaluate, diagnose, and provide initial treatment to adult patients who present in the Emergency Department with any symptom, illness, injury, or condition; to provide services necessary to ameliorate minor illnesses or injuries and stabilize patients with major illnesses or injuries; and to assess all patients to determine whether additional care is necessary. Perform procedures related to Emergency Medicine such as: Basic and advanced airway management, rapid sequence intubation, management of mechanical ventilation, administration of moderate sedation, EKG interpretation, cardiopulmonary resuscitation, cardioversion, defibrillation, transcutaneous cardiac pacing, venous central line placement, intraosseous access, peripheral arterial puncture for ABGs, NGT placement, gastric lavage, gastrostomy and tracheostomy tube replacement, reduction and immobilization of fractures and dislocations, wound care and closure {suture), abscess incision/drainage, lumbar puncture, paracentesis, knee arthrocentesis, anterior/posterior nasal packing, peripheral vein catheterization, vaginal pelvic exam and Foley Bladder Catheterization, Emergency surgical procedures indicated by emergency medical conditions or injuries such as emergent cricothyrotomy, chest tube placement and suprapubic cystostomy. Emergency Medicine physicians may admit to inpatient services only on special circumstances. Participation in the coordination of interfacility transfers for patients requesting transfer from another emergency department to the VACHS emergency department. Participation in the coordination and recommendation for appropriate destination of patients in the out-of-hospital environment requesting access to the VACHS emergency department. Active participation in the coordination of efficient patient flow in the emergency department. Active participation in the activation and deactivation of Hospital Diversion mode. Ensures that high quality patient care is delivered in a resource efficient manner. Promotes an environment of system-oriented continuous quality improvement. Establishes and maintains effective harmonious relationships with other peers within Medicine and other Services within our station. Responsible for the supervision of assigned residents and students rotating in the Emergency Department, which include review of medical records, patient care, treatment, and consultation requests. Responsible for the supervision of Intermediate Care Technicians (ICT) in the ED, including supervision of procedures and co-signature of notes. Follow all duties designed by the Supervisor and/or Service Chief that could be changed within a reasonable time and within an internal medicine clinical setting being provided appropriate instructions and coordination. Ensures that high quality patient care is delivered in a resource efficient manner. Manage patients with Critical Care Medicine related conditions such as but not limited to: acute and chronic respiratory failure, obstructive and restrictive airway disease, home oxygen dependency problems, the patient in shock of diverse origin, severe sepsis, obstructive sleep apnea, circadian rhythm misalignment and others. Perform procedures related to the Critical Care Medicine subspecialty such as but not limited to : central line placement, interpretation of hemodynamic data, nasotracheal/orotracheal intubation, sonographicatly directed central line placement, prescription of oxygen therapy, positive airway pressure, evaluation of imaging studies, etc. Active, dynamic and up-to-date participation in medical research related to critical care medicine/ emergency medicine related conditions. Participation in educational and scholar activities such as seminars, journal clubs, grand rounds, tumor boards, treatment planning conferences and dissemination of acquired knowledge through peer reviewed publications. Complies with VHA regulations and follows professional standards and ethical conduct. Work Schedule: Monday through Friday, 8:00 am to 4:30 pm, rotating shifts and on-call as needed.
