You Will Be Responsible For:
Responsible for the Marketing/Business Development of the Cardiopulmonary Rehabilitation Program
Understands Cardiopulmonary Rehabilitation business model. Explores and reaches out to community physician practices/groups, build relationships to engage those practices/groups in the program, thus growing outpatient market share.
Researches and develops tools and strategies to promote the Cardiopulmonary Rehabilitation Program to area physicians.
Visits physician offices regularly. Handles physician/physician office issues in a timely manner. Initiates physician communications as needed.
Assures physician satisfaction with services and presents to the director(s) any concerns to be reviewed on an ongoing basis. Meet with Director of Rehabilitation Services monthly.
Responsible for programmatic development in collaboration with the Director of Rehabilitation Services in establishing, implementing and evaluating goals related to growth, volume, clinical outcomes and revenue.
Facilitates the navigation of patients who qualify for outpatient Cardiac or Pulmonary Rehabilitation, as the single point of contact, through the program. Analyzes, trouble shoots and makes recommendations for ways of improving services and areas that may have problems.
Establishes and maintains positive and open communication (verbal and written) and relationships with
referring physicians, pulmonologists, surgeons, cardiologist, and cardiothoracic surgeons to provide quality care to patients with disease specific health concerns. Ensures communication with referral physician happens through each stage of the patient's treatment, without fail.
Facilitates patient continuity of care between hospital departments and provides patient follow-up for Cardiac and/or Pulmonary Rehabilitation. Timely identification and resolution of issues are handled effectively and in a timely manner; as it relates to the delivery of patient health care and physician
Coordinates and participates in cardiopulmonary meetings, with the CHF navigator, and the Chest Pain coordinator.
Serves as a resource to cardiac and pulmonary patients as it relates to health concerns. Teaches and answers questions about disease specific health concerns, provide and/or participate in health lectures and educational outreach programs, health fairs, corporate affiliated events; available for television, print and radio marketing, as needed. Represents ADH Ocala in the community as the "Cardiopulmonary Rehabilitation Ambassador".
Educates patients concerning what can be expected prior to, during and after cardiac/pulmonary rehabilitation. Develops and revises patient educational materials. Provides staff and community education as required.
Evaluates the patient's physical and emotional recovery after treatment, utilizing appropriate resources
and/or develops and facilitates program support groups. Demonstrates awareness of patient's needs as a total person (physical, spiritual, emotional, intellectual, cultural, etc.), consistent with patient's age and development level.
Assists research staff in identifying potential patients for hospital-based research protocols.
Communicates timely in a clear concise manner and presents each day in a professional manner.
Reviews medical management in regard to evidence-based practice and in accordance to national guidelines; focusing on improving outcomes. Responsible for the ongoing data input/data abstraction/data analysis of ongoing databases. Monitors data tracking and benchmarking in collaboration with the Program Medical Director(s) and Administrative team
Assist as needed.
What You Will Need:
Strong interpersonal skills, excellent conflict resolution and communication skills and computer proficiency required.
Must have or establish positive working relations with the medical community in Marion county.
Leadership, mentorship/clinical resource skills
Ability to multitask on a variety of tasks and projects
Must have knowledge of The Joint Commission requirements and local, state and federal regulations.
Must have knowledge on AACVPR and current legislation impacting provision of Cardiopulmonary Rehabilitation Services.
Exercise Physiologist with cardiopulmonary rehabilitation experience
Minimum of 2 years healthcare experience
Phase I, II, and III experience preferred
Healthcare marketing and business development work experience preferred
Driver's license required
Current certification in ACLS required
Current valid State of Florida licensure required
Under the direction of the Director of Rehabilitation Services, and in collaboration with the OP Rehab Manager and OP Rehab Supervisor; the Cardiopulmonary Rehabilitation Program Coordinator is responsible for the AACVPR Certification for both cardiac as well as pulmonary process including data collection for outcome measures, staying up to date on evidence based guidelines, educating staff regarding guidelines/processes which are required for certification, developing competencies, develop policies/procedures in accordance with AACVPR, and any task relating to the certification/maintenance of the Cardiac and Pulmonary programs. The overall goal is to ensure the provision of high-quality rehabilitation services to our patients. Responsible for establishing, building and maintaining strategic relationships between our community physicians and their office staff. Responsible for promoting, educating and marketing the Advent Health Cardiopulmonary Rehabilitation Program to employed physician, community physicians and their office staff.
